What Is It?
The Sustainable Outcomes Service (SOS) is Transition2’s bespoke follow-on employment support service for young adults aged 18–25 with SEND who are already in paid work.
Securing a job is a significant achievement, but sustaining employment and progressing in the workplace can bring new opportunities and challenges. SOS provides flexible, tailored support to help young adults remain successful in work, build confidence and develop greater independence.
Operating on a roll-on, roll-off basis, support is responsive to individual need and may include:
- Workplace coaching and mentoring
- In-work support and guidance
- Performance and wellbeing reviews
- Employer liaison and workplace problem-solving
- Job-specific training and skills development
- Support with career progression and changing job roles
Working in partnership with employers, families and professionals where appropriate, SOS provides timely, targeted intervention that helps sustain employment, reduce the risk of job breakdown and enable young adults to develop fulfilling, long-term careers.
