Sam

Sam’s end goal is to “Become a more responsible and independent young man to gain a local voluntary job.”

Since joining us, Sam has worked hard to develop his self-management skills in order to increase his understanding of the consequences of his actions, and to build independence. He has become more independent at home; getting himself ready for activities with few prompts, and has also transferred the independent living skills learned at college to help at home.

Sam initially struggled to look after his belongings, in particular his wallet. With the help of a prompt sheet to remind him what he needs for the day, he is now able to take responsibility for his own belongings and goes independently to the bank.

It was important for Sam to become more independent and his successful travel training over the last year has equipped him with the skills and confidence to travel independently from his home into town and back. He uses his mobile phone to inform family and staff where he is on his journey and this contact has further increased his capacity to be safe in the community.

Sam has achieved his end goal as he has secured a regular voluntary role at Morleston Day Centre where he prepares the tables for lunch and clears up afterwards for the elderly people. He walks independently to Morleston, following a walking route he had been taught.

Now that he is an independent traveller, he chooses to meet his friends at the gym before having lunch at a local café. He has become known to the staff and this connection with a place in the community and local people means that the cafe has become a safe place for Sam to go if he needs support.

In the future Sam is hoping to continue enjoying his independence to access the gym, carry out his voluntary work and meet his friends.
